POSITION: Infection Preventionist/Wound Care Nurse

SUMMARY:
Infection Preventionist
The Infection Preventionist reports to the Director of Nursing and partners with the Medical Director, Medical Director for Antimicrobial Stewardship, the Quality Officer, and other stakeholders to develop a system of care that promotes sound and scientific infection prevention principles and practices.

This individual is accountable for decreasing the incidence and transmission of infectious diseases between patients, staff, visitors and the community. Through strategic planning, leadership and consultation, you will lead and direct a robust team in the identification and implementation of infection prevention goals and objectives throughout the facility.

Wound Care
Coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation and outcomes for patients throughout the organization. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Is a resource person for physicians wound team and clinical staff. Additionally is able to perform general nursing duties in all departments with adequate supervision. Participates in performance improvement plan of the organization.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.


Infection Preventionist
• Partners with the Medical Director for Epidemiology/Infectious Disease to develop, implement and evaluate annual infection prevention goals and action plan
• Partners with facility leaders, physicians, local, state, and national agencies on activities related to infection prevention
• Oversees the operations of the infection prevention, epidemiology, industrial hygiene, and relevant safety programs
• Accountable for surveillance of healthcare acquired and community acquired infections
• Manages goal setting process for system, unit and physician goals related to healthcare acquired infections and conditions
• Lead industrial hygiene program to anticipate, recognize, evaluate, mitigate and control workplace conditions
• Participates in the national collaboratives and external reporting to CDC NHSN system, and other post acute-care specific quality organizations
• Authority and responsibility for ensuring appropriate intervention and education occurs with staff, volunteers and medical staff when healthcare infection trends, outbreaks or non-compliance to infection control/OSHA are identified.
• Ensures that education and counseling on infection prevention is available for staff, volunteers, medical staff, patients and parents.
• Support and participate in interdisciplinary performance and quality improvement process.
• Maintains current knowledge of federal, state and local regulations and ensures that the facility leaders are informed of appropriate issues. Understands and complies with infection control, safety and OSHA procedures and regulations.

Wound Care
• Ability to perform wound assessment, reassessment and management for all patients and per policy.
• Knowledge of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.
• Utilizes principles of current wound management theory when providing care to patients with acute and chronic wounds.
• Demonstrates thorough knowledge of staging system of wounds.
• Develops skin care programs to prevent skin breakdown of patients.
• Knowledgeable of surgical procedures and postoperative course of patients undergoing bowel surgery.
• Demonstrates knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care and incontinence.
• Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to interpret data about the patients status in order to identify each patients age specific needs and provide care needed by the patient group.
Ability to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan for effectiveness.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Demonstrates ability to perform treatments and provide services to level licensure.
• Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
• Provides education to all patients undergoing surgery that result in an ostomy. Provides pre and post-op assessments and patient/family education.
• Knowledgeable of medications and their correct administration based on age of the patient and clinical condition.
• Is a resource person to physicians, nursing and other hospital staff. Provides in-services on wound prevention, wound care, ostomy care and incontinence for the nursing staff.
• Communicates appropriately and clearly to nurse managers, nursing staff and physicians.
• Consults other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the patient's needs.
• Documentation meets current standards and practices.
• Integrates research findings to clinical practice.
• Attends seminars, conferences, continuing education classes in the areas of wound prevention, wound management and ostomies. Participates in professional societies. Stays current with changes, new advances/treatment in wound care.
• Serves as a liaison between physicians, patients and manufacturers of clinical products.
• Knowledgeable of the clinical products currently available on the market Meets with sales representatives, evaluates products; makes recommendations for products/equipment.
• Operates all equipment correctly and safely.
• Coordinates and supervises patient care as necessary.
• Demonstrates an ability to be flexible, organized and function under stressful situations.
• Inputs charges and enters documentation on all wound care patients into computer system in a timely manner to ensure compliance.

QUALIFICATION REQUIREMENTS:

EDUCATION and/or EXPERIENCE:
• Graduate of an accredited school of nursing with current Nebraska licensure as a Registered Professional nurse, epidemiology, microbiology, medical technology, public health, or other healthcare science
• 3-5 years licensed experience in healthcare setting, preferably in long term care.
• Employee performs within the prescribed limits of the hospitals/departments Ethics and Compliance Program. Is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance officer or hospital hotline.
